- Complete Questionnaire and Consent Forms: Our staff will provide you with a questionnaire, along with additional information about our programs. This will help us understand your symptoms and needs. Consent forms will also be shared, which can be conveniently completed electronically on our secure platform.
- Personalized Initial Comprehensive Health Assessment: All patients are required to attend a personalized initial comprehensive assessment at our clinic. During this visit, we will assess your health, review your medical history, and discuss your symptoms in detail. We will also order any necessary lab work and collect specimens in our clinic. We will use the results of your lab work to create a personalized treatment plan tailored to your specific needs.
